Here is the soft copy of the health declaration form. Please print it out, get it signed and return it to us by Saturday.

Also do take note that Saturday's training will start at 9am sharp. Remember to be dressed in your sports attire and sports shoes and bring along your water bottle (at least 500ml) and small towel. Hope to see all of you on Saturday!
